PCAAT helps connect people to primary care while helping them overcome barriers to accessing care like housing instability, poverty or refugee status. The team serves individuals aged 16+ or family units who are experiencing complex health and social needs related to trauma, substance use, social isolation, displacement, and mental health concerns. In addition to connecting to primary care and coordinating with other community resources, the team offers mental health counselling, peer support, advocacy and nursing and nurse practitioner services.
