The Hamilton Midwifery Outreach Team provides provisional, mobile, antenatal and postpartum care, to homeless or precariously housed individuals. Midwives provide care in keeping with the principles of harm reduction and endeavour to support clients with a transition into a community stream of care.
The Hamilton Midwifery Outreach Team has a paging system and an on-call midwife available to provide non-emergent care when needed. Access to the program is coordinated through front-line workers and partner agencies. A front-line shelter, service or healthcare worker can page the Hamilton Midwifery Outreach Team midwife when they are in contact with a pregnant or postpartum person who wants to be seen by a midwife.
When the on-call outreach team midwife is paged, they will make every effort to meet the client in a timely fashion at the location of the client’s choosing, within reason. If the midwife is not able to attend they will make an effort to have the client seen by another midwife, or as a last resort the midwife will make an appointment to see the client at the earliest possible opportunity.
Care is delivered by midwives in collaboration with Shelter Health Network physicians.
If you are a front-line shelter, service or healthcare worker looking to reach the on call midwife for the Hamilton Midwifery Outreach Team, the pager is: 905-561-0738
