About us

We provide accessible, low-barrier health care in partnership with shelters, housing providers, social service agencies, and community organizations throughout Hamilton.

Who We Are

The Shelter Health Network is a collaboration of health care professionals and community organizations working together to improve access to care for individuals experiencing homelessness or housing instability.

Our patients often face significant barriers to accessing traditional health care services, including:

  • Lack of identification
  • Limited access to phones or computers
  • Transportation challenges
  • Previous experiences of stigma or discrimination

By delivering care directly within community settings, we aim to meet people where they are — both literally and figuratively.

Our Approach

We provide accessible, low-barrier health care in partnership with shelters, housing providers, social service agencies, and community organizations throughout Hamilton.

Our goals are to:

  • Improve access to primary health care
  • Support continuity of care
  • Reduce unnecessary Emergency Department use
  • Improve health and quality of life
  • Promote social stability and wellbeing
